DNR: Do Not Reciprocate is a yandere-flavored romantic visual novel where you play as Niko, a sickly young man who has been hospitalized numerous times. During this stay Niko meets three doctors, each in a distinct specialty. The initial check-ups appear regular, but the doctors’ attention in Niko soon becomes strange.

Sage, the Neurologist

Sage is a brain specialist and one of three persons with whom Niko can build a relationship. Meetings with Sage provide the dual purpose of monitoring Niko’s status and providing an opportunity for personal interaction. Sage’s concern for the patient borders on obsession and the line between concern and something else gets blurry.

Amia, the Cardiologist

Amia specializes in cardiology and has her own devoted story line. Niko’s health problems provide the excuse to keep meeting, and the way Amia expresses her worry gets more and more intimate.

Robin, the Ophthalmologist

He is the third doctor Niko visits at the hospital. Robin, like the other two characters, has a plot thread with dialog options that lead to distinct conclusions.

DNR: Do Not Reciprocate and Niko's Decisions

In DNR: Do Not Reciprocate the gameplay involves reading dialog and selecting responses. Players follow Niko’s tale, talk to the physicians and make decisions at key points. Each doctor has a different story with two possible ends, thus there are a total of six possible endings. The game also contains 7 CGs and more than 17,000 words of material.

All three of the game's relationships have yandere overtones; thus, the affection exhibited to Niko does not always stay within the confines of normal affection. The story deals with scenarios of control, obsession and transgressive behavior in a medical setting.
